Comes with the next-gen update's Forgotten Wolven gear. Reinald hands the diagram over at the end of 'In the Eternal Fire's Shadow' however you resolve it; the quest starts with a priest by a wagon on the road east of Mulbrydale in Velen. If you already finished it, Osmund's notes on the lootable bookshelf at Kaer Morhen unlock the diagrams too -- reach it by the ladder near the scaffolding at the back of the main hall.
